Blepharitis: Understanding Inflamed Eyelids
Do you experience inflamed, red, swollen, or crusty eyelids? If so, you might have undiagnosed blepharitis. Blepharitis is the medical term for inflammation of the eyelid. Common symptoms of blepharitis include swollen/puffy eyelids, redness, itchiness, burning sensation, and accumulated crust/flakes around the eyelashes. Additionally, depending on the severity of the condition it can result in blurred vision, light sensitivity, dry eyes and loss of eyelashes.
Similar to normal skin microbiomes, eyelids also house microorganisms. These microorganisms aid in maintaining a barrier of protection; however, some patients can have an imbalance in their microbiome which increases their risk of developing blepharitis. Oily skin, clogged oil glands, poor lid hygiene, and inflamed/infected/irritated skin can all result in increased bacteria and thus blepharitis. In addition to an imbalanced microbiome, contact lens wear, makeup debris, hormonal changes, and dry environments are also risk factors for blepharitis. Blepharitis is differentiated into either anterior or posterior blepharitis depending on the etiology and which part of the eyelid is affected. Causes of anterior blepharitis include acne rosacea, seborrheic dermatitis (dandruff/flakes of lid/lashes), dry eyes, and demodicosis (mites in the lashes). Causes of posterior blepharitis include acne rosacea, seborrheic dermatitis (dandruff/flakes of lid/lashes), and meibomian gland dysfunction.
Blepharitis is diagnosed with a thorough case history and slit lamp evaluation. Treatment of blepharitis can vary depending on the cause; however, the first line of treatment includes eyelid hygiene, warm compresses, and lid massages. Lid scrubs or baby shampoo can be used to gently clean the eyelids and eyelashes to remove debris and crusting. After cleaning the eyelids, we recommend warm compresses followed by lid massages to promote healthy oil expression. In cases of severe, or chronic, blepharitis it may be necessary to start antibiotics or topical steroids. Based on the exam findings we will create a tailored treatment plan to help treat and manage the condition.
